Job Overview

Shriners Children's Chicago has an opportunity for a RN Care Manager to join Care Management team. Join a team of professionals committed to improving children's lives. Responsibilities

  • The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care.
  • Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience.
  • You'll help to provide continuity of care for positive outcomes in our family centered environment.
This is not an all-inclusive list of this job's responsibilities. The incumbent may be required to perform other related duties and participate in special projects as assigned.

Qualifications

Required:
  • BSN
  • 5+ years of clinical experience in Pediatrics, Spinal Coord Injury and/or Orthopaedics required.
  • 3+ years nursing required.
  • RN Illinois Licensure required.
  • 1+ years Care Coordination/Case Management experience in Pediatrics is preferred.
  • Case Management Certification (CMC, ACM or ANCC) required within 12 months of hire

  • MSN
  • Bilingual English/Spanish
