Job Description

Reporting to the Director of Pharmacy and/or their designee, the Clinical Pharmacist is responsible for optimizing patient outcomes through the effective management and implementation of medication adherence programs and ensuring accurate and efficient medication management across our patient care continuum. Seek additional business opportunities to partner with and increase program profitability. The ideal candidate will have a background in pharmacy or health administration, experience with clinical or community pharmacy settings, and a passion for improving health access and outcomes in underserved communities.

Essential Job Functions
• Develop and implement medication adherence and medication reconciliation programs that are tailored to the needs of our diverse patient population

. • Collaborate with healthcare providers, including physicians and pharmacists, to identify patients at high risk of non-adherence and develop personalized intervention strategies

. • Utilize data analytics to monitor adherence rates and assess the effectiveness of interventions; adjust strategies as necessary to meet adherence goals

. • Educate patients and their families about medication management, including the importance of adherence, side effects management, and the proper use of medication devices

. • Provide inservice to pharmacy and clinic staff about relevant clinical topics as needed
• Conduct regular follow-ups with patients via phone calls, or telehealth services to reinforce adherence strategies and troubleshoot any barriers to compliance
• Coordinate with pharmacy staff to simplify medication regimens where possible, including the synchronization of refill dates and the use of pill organizers or blister packs

. • Design and manage medication adherence and reconciliation initiatives and protocols

. • Analyze program effectiveness and make data-driven adjustments

. • Utilize electronic health records (EHR) and other technology tools to track and manage medication information effectively
• Working with senior-level stakeholders and clinical leaders to develop, implement, and execute successful clinical pharmacy initiatives

. • Managing rapid change management, continuous improvement, and process standardization initiatives • Fostering a culture of excellence and engagement for a scaling clinical team
• Load balancing continued clinical operations, pilot programs, and health plan initiatives

. • Ensuring excellent and compliant customer service is provided to patients

. • Executing a performance management framework that enables sustainable and efficient operations

. • Reporting on your metrics and initiatives to senior leadership on a weekly basis
