POSITION SUMMARY

The Nurse Care Coordinator advocate and cares for patients and utilizes professional nursing theory and the nursing process to implement, coordinate and direct patient care and auxiliary nursing care activities utilizing the PCMH model of care. This role will work with Primary Care patients.

LPN pay range - $53,077/year - $63,693/year

RN pay range - $70,195/year - $84,233/year

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Maintains tracking in databases, Excel, and any other documentation regarding patient care issues
  • Performs all nursing support duties for the Behavioral Health department
  • Conducts daily nurse visits and triage support for Primary Care, including administration and tracking of select injectable treatments
  • Manages infection control and needle stick incident program
  • Provides specialized patient education as needed in individual or group settings
  • Develops care plans utilizing both patient identified and health team recommended individual goals
  • Adheres to health center safety, infection control, and hazardous waste procedures
  • Maintains patient confidentiality at all times in accordance with HIPAA regulations
  • Provides individual provider nurse care support for at least one provider, if indicated
  • Acts as a patient advocate
  • Contacts patients on monthly basis, monitor logs, and progress
  • Provides patient support and education through direct patient care, including patient teaching, patient treatments, nurse visits for assessment and triage, phone triage and acts as clinic resource to patients, providers, and other clinical staff
  • Identifies and addresses patients with risk factors that impact chronic illness
  • Manages and/or participates in health center programs such as SMHW, VFC, Ryan White, Infection Control, STD disease monitoring, PCHH or specific disease monitoring and impact such as diabetes, hypertension, hepatitis C or HIV (not exhaustive list).
  • Participates in Quality Improvement processes and activities in the organization.
  • Utilizes MHD (Mobile access to Health Documents) technology programs and initiatives
  • Monitors HIT (Health Information Technology) tools and reports to identify gaps in care and needed services
  • Addresses medication alerts, hospital admissions and ER visits

MINIMUM REQUIREMENTS

  • LPN degree/certification from an accredited school of practical nursing, or
  • RN degree, ADN, BSN, or higher
  • Must be licensed as a Licensed Practical Nurse or Registered Nursing in accordance with state regulations
  • Minimum of one-year experience in a hospital or clinic setting

  • Basic Life Support (BLS) Certification

PREFERRED REQUIREMENTS

  • Ability to multi-task and handle a variety of demands
  • Experience with Microsoft Office products
  • Experience with ECW

Working Conditions

General working conditions are in a healthcare facility. As such, there is a potential exposure to virus, disease, and infection from patients while performing the duties of this job. May experience traumatic situations including psychiatric traumatized and deceased patients. Travel may be expected to community meetings, client homes, or other agencies.

Physical Demands

While performing duties of this job, employees are regularly required to set, walk, and stand; talk and hear; both in person and by telephone; use hands repetitively to finger, handle, feel, or operate standard office or clinical equipment; reach with hands and arms; and occasionally lift up to 20 pounds.
