SUMMARY OF POSITION

The HIV Medical Case Manager facilitates and provides a range of holistic, client-centered services to persons living with HIV, with the goal of streamlining access to medical care to achieve durable viral suppression for each client; addressing barriers to care and social determinants of health; and collaborating with multidisciplinary teams including Vibrant Health clinical and non-clinical staff, local agencies, and community partners, to optimize coordination and continuity of care for clients and patients living with HIV.


ESSENTIAL JOB DUTIES

Provide HIV medical case management services, in compliance with state and agency standards, to persons living with HIV (PLWH), inclusive of the following:

1) Employ HIV case management services in accordance with HRSA’s Ryan White HIV/AIDS Program standards and guidelines, to PLWH through continuous client assessments, service planning, and service plan monitoring;

2) Determine client eligibility for HIV medical case management services;

3) Perform intakes and comprehensive psychosocial client assessments, and assist with insurance enrollment as needed, either directly or through referral;

4) Develop thorough and individualized client-centered service plans that promote effective and efficient delivery of medical care;

5) Coordinate referrals and client access to appropriate short- and long-term housing programs, behavioral health services, transportation and food assistance, legal and employment resources, and additional social service programs as appropriate;

6) Conduct client monitoring and reassessments for efficacy of established service plans;

7) Supply clients with ongoing advocacy, education, support, and assistance throughout every stage of the HIV Care Continuum;

8) Maintain accurate client files and timely documentation of services being provided utilizing the patient electronic health record, specialized HIV medical case management software (SCOUT), and other relevant databases as appropriate;

9) Attend and participate in patient appointments, staff meetings, client case conferencing, in-service trainings and workshops, local outreach events, and department, agency, and community partner meetings;

10) Establish collaborative working relationships with medical, dental, and behavioral health service providers as well as appropriate community resource providers, including but not limited to the local health departments, agencies providing HIV testing, Linkage to Care, and other social service organizations as appropriate.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

1) Bachelor’s or graduate degree in social work, health care, or related human services major from an accredited college or university;

2) Minimum of one year experience providing direct services to clients and/or patients in the community- or population-based clinic;

3) Strong knowledge of medical, behavioral health, and social service systems;

4) Cultural humility; ability to provide appropriate, compassionate, high-quality care to a culturally and linguistically diverse client population with a range of backgrounds and lived experiences; knowledge of and sensitivity to PLWH and intersecting communities disproportionately affected by HIV;

5) Willingness and commitment to maintain a robust working knowledge of best practices related to the HIV Care Continuum and service provision;

6) Highly developed client-centered communication skills; strong written and verbal skills and ability to produce timely and detailed documentation.

PREFERRED KNOWLEDGE, SKILLS, AND ABILITIES

1) Previous experience working as a Medical Case Manager serving PLWH.

2) Bilingual fluency (Spanish/English);

3) Familiarity with Ryan White Part A and Part B.

4) Lived experience with HIV, or as part of an intersecting community disproportionately affected by HIV.

5) Advanced knowledge of psychosocial and/or clinical aspects of the HIV Care Continuum and service provision.
