Responsible for improving the health of patients dwelling in their own homes. This requires an individualwho is comfortable working independently, enjoys teaching, is a critical thinker and has strong communication skills. Assesses the needs of the patient and coordinates the care with an interdisciplinary team. Provides hands on care in the home environment and offers autonomy, flexibility and the luxury of caring for one patient at a time. Experience in home health preferred. Ideal candidate possesses excellent interpersonal communication skills. Strong analytical and PC skills. Experience with pre-acute and post-acute care as well as community resources. Ability to work independently and collaboratively with care providers and business office staff is necessary. Promotes safety, cost efficiency and a commitment to quality improvement. The qualified candidate will possess the following: Current and valid State of WI CNA license or ability to obtain within 90 days of hire Experience in home health preferred EPIC proficiency desirable Current and valid driver\'s license and proof of insurance coverage as required by ProHealth.
