LVN Refill Coordinator

Sante Health System is in search for a Licensed Vocational Nurse to oversee and coordinate the prescription refill process for Community Medical Providers. This role will be responsible for overseeing and coordinating the prescription refill process. The LVN will use a systematic approach to refill management, while applying all available EMR functionality and assuring the process is following written CMP protocols. The LVN will communicate with office staff and as needed, with patients and pharmacies, to assure medication refills are completed in a thorough and efficient manner. The position will also support continuous process improvement of the refill management workflow.

QUALIFICATIONS

  • Graduate from an accredited LVN program.
  • Two - three years of LVN experience.
  • Must possess a current California LVN license.
  • Strong relationship and customer service skills are required.
  • Effective oral and written communication skills are a must.
  • EPIC or other medical office software experience is preferred.
  • Word processing experience required.
  • Experience with medical terminology required.
  • Prescription refill processing experience is beneficial.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Support the development of CMP-wide medication refill processes and applying available EMR technology.
  2. Support the continuous identification and improvement to the medical refill workflow.
  3. Receive, review and confirm all medication refill requests.
  4. Review patient charts for all incoming requests for review of prescription history.
  5. Per protocol, approve medication refills for patients.
  6. If needed, communicate with patient on concerns relating to prescription medications and refills.
  7. If needed, make or assure that appointments are made.
  8. Document refill information in charts, or file appropriate denials and/or changes in
  9. Refill turn-around is 24 to 48 hours from receipt of request whether verbal, fax or
  10. Develop processes or reports to determine overdue medications.
  11. Answer patient and pharmacy calls regarding medication refills.
  12. Maintain patient files, records and other information.
  13. Maintain patient confidentiality.
  14. Provide excellent customer service.
  15. Provide support, as needed, to any internal or external audit requests related to medications.
  16. Attend meetings as required.
