Housing Navigator: Rise

Engages clients experiencing homelessness who enter the Rise & Thrive program and assist in providing client-centered services. May assist case managers with developing, organizing and conducted programs to resolve challenges associated with homelessness, and securing housing.

ESSENTIAL AND MARGINAL FUNCTION STATEMENTS

Essential and other important responsibilities and duties may include, but are not limited to, the following: team scheduling, training, coaching and other leadership tasks.

Essential Functions:

  1. Performs initial assessments to clients
  2. Ensures clients have at minimum a homeless identification card when entering campus.
  3. Navigates clients to partner service providers and in-house case managers.
  4. Operates client computer areas.
  5. Assists clients in securing housing
  6. Collects and provides client information in accordance with established state and federal requirements for confidentiality; and maintains client files regularly.
  7. Maintains a variety of automated and manual records and files; provides operational information and statistical data for management use.
  8. Coordinates job-related activities with professional colleagues.
  9. Attends and participates in group meetings with peers. Stays abreast of new trends, programs, and innovations.
  10. Attends professional training.
  11. Duties are varied in nature and require independent judgment to determine the proper course of action within the limits of policy and procedure.
  12. General objectives are established, and the employee is required to select their own method of attainment, referring unusual cases to the agency supervisor.
  13. Evaluates existing procedures, recommends changes.
  14. Conducts interviews with clients, family members, service providers, and others to obtain information for formulating program/service eligibility and case status.
  15. Identifies social, economic and physical needs of clients.
  16. Assesses client's support systems, available community resources and other factors to plan, develop, and implement an appropriate plan.
  17. Utilizes agency guidelines, state, and federal regulations to determine/confirm eligibility for programs and services.
  18. Refers clients to appropriate community medical, emotional, economic and social support organizations; advocates for or assists the client in obtaining such services.
  19. Prepares complete and accurate case notes.
  20. Writes correspondence, reports and other written materials.
  21. Uses standard office equipment, including a computer, during work.

QUALIFICATIONS

Knowledge of:
  • Knowledge of available community services and program schedules onsite
  • Basic principles and practices of community program practices.
  • Principles, practices and techniques of customer service delivery.
  • Basic computer functions, including use of common office software such as word processing, spreadsheet, and database application.
  • Principles and procedures of record keeping.
Skills in:
  • Dealing successfully with a variety of individuals from various socioeconomic, ethnic, and cultural backgrounds, in person and over the telephone.
  • Communicating clearly and concisely, both orally and in writing.
  • Establishing and maintaining cooperative-working relationships with those contacted during work.
  • Administering data tracking tools utilizing computer software.
  • Ability to learn the geography and demographics of the city.
  • Effective communication, both orally and in writing, including public presentations to groups.
