VT Pathways to Partnership Transition Coordinator

The State Of Vermont is looking for a Transition Coordinators to work with the Vermont Pathways To Partnership Grant Program, helping students with disabilities become self-advocates and primary decision makers to their futures in secondary education and employment
You'll work with VT P2P team leaders, local schools and partner organizations from the disability community, to implement regional programs and help placement efforts for youths with disabilities.
Other duties include:
- Coordinating services for students with disabilities & their families
- Data Collection & presentation
- Providing technical assistance to local education agencies
- Ensuring budget and details of tasks are organized
- Maintain relationships with contacts at regional schools
There are 3 open positions, one for each region in the state, Northern, Central and Southern. While this position will be located in a local regional office, significant fieldwork within the region and statewide is expected, and a reliable means of private transportation is required. Candidates should have considerable understanding of a variety of disabilities and their impacts on education and employment.

Duties are performed in both field and standard office settings. Considerable travel throughout the assigned district is required, for which private means of transportation must be available. Stressful situations should be anticipated, and incumbents must be able to persevere in difficult or problematic circumstances. Some work outside of normal office hours may be required by client needs.
Minimum QualificationsTwo (2) or more years of experience working in a human services field.
Associates degree or higher in human services, psychology, social work or special education.
Experience working with disabled youth.
Special RequirementsCandidates must pass any level of background investigation applicable to the position. AHS Policy 4.02, Hiring Standards, pursuant to 20 V.S.A. 2056c or other applicable statutory authority, requires criminal record checks for all AHS positions, including motor vehicle driving record checks and national record checks where appropriate.
