Job Summary:

The Clinical Director is responsible for overseeing and coordinating the clinical services provided by the Home Health Care Agency. This individual ensures compliance with federal, state, and local regulations, as well as internal policies and procedures. The Clinical Director leads the clinical team to deliver high-quality patient care and is instrumental in the development and implementation of patient care strategies that meet the agency's goals and objectives.

Key Responsibilities:

  1. Clinical Oversight:

Supervise and coordinate the delivery of clinical care to patients in their homes.

Ensure all care provided complies with state and federal regulations, including Medicare guidelines and accreditation standards.

Lead quality improvement initiatives to enhance patient outcomes and improve clinical practices.

Conduct regular audits of clinical documentation and ensure compliance with internal policies.

Serve as the primary point of contact for clinical concerns and escalations.

  1. Staff Management and Development:

Recruit, train, supervise, and evaluate clinical staff, including nurses, physical therapists, occupational therapists, speech therapists, and social workers.

Develop and implement orientation and ongoing training programs for clinical staff.

Provide leadership and mentorship to the clinical team, fostering a positive and productive work environment.

Evaluate staff performance regularly, ensuring appropriate competencies are met.

Address performance issues and implement corrective actions when necessary.

  1. Patient Care Management:

Collaborate with physicians, other healthcare providers, and families to coordinate patient care plans.

Monitor patient outcomes and adjust care plans as necessary to achieve optimal health results.

Address patient and family concerns regarding clinical care and services.

Ensure that patient care is individualized and meets the diverse needs of the population served.

  1. Policy and Procedure Development:

Develop, update, and implement clinical policies and procedures to ensure safe, effective, and consistent care.

Stay current on healthcare regulations and implement necessary changes in clinical practices.

Ensure all staff are knowledgeable about and adhere to agency policies and procedures.

  1. Compliance and Quality Assurance:

Monitor compliance with all clinical documentation, billing, and care standards.

Work with the quality assurance team to analyze clinical data and implement improvement strategies.

Lead agency efforts to achieve and maintain accreditation status.

Manage the agency’s infection control program and ensure compliance with health and safety standards.

  1. Collaboration and Communication:

Collaborate with the agency’s administrative and operational leadership to align clinical goals with the overall agency strategy.

Attend and lead regular clinical team meetings, case conferences, and other staff meetings.

Communicate with external stakeholders, such as physicians, payers, and regulatory bodies, to ensure coordinated and compliant care delivery.

Qualifications:

Education: Bachelor’s or Master’s Degree in Nursing or a related healthcare field.

Licensure: Must hold a current, valid RN license in the state of practice. Advanced certifications in healthcare leadership or home health are preferred.

Experience: Minimum of 3 years of clinical experience in home health care, with at least 2 years in a leadership or supervisory role.

Knowledge: Deep understanding of home health care regulations, Medicare guidelines, accreditation standards, and clinical best practices.

Skills: Strong leadership, problem-solving, and communication skills; ability to manage multiple priorities and work under pressure; proficiency with electronic medical records (EMRs) and documentation standards.
