  • Direct Patient Care: Conduct comprehensive assessments, manage chronic conditions, and provide preventive care services for residents, ensuring continuity of care as their primary physician.
  • Routine Rounds: Perform regular rounds at assigned facilities, addressing both chronic and acute health concerns.
  • Care Coordination: Collaborate with facility staff, family members, and interdisciplinary teams to create and manage personalized care plans tailored to each resident's needs.
  • Documentation and Compliance: Ensure accurate, thorough documentation in the EMR system, adhering to regulatory requirements and Curana Health's policies.
  • Patient Education: Educate residents and their families on health management, preventive care, and available resources.
  • Quality Assurance: Participate in quality improvement initiatives, analyzing performance metrics to continually enhance patient outcomes.
Qualifications
  • Medical Degree (MD or DO) from an accredited institution
  • Board Certification in Family Medicine, Internal Medicine, or Geriatrics
  • Active, Unrestricted Medical License in the state of practice
  • DEA Certification and current BLS/ACLS certification
  • Proficiency in EMR systems and timely documentation practices
  • Strong Interpersonal and Communication Skills, with a commitment to patient-centered care
Why Curana Health?
  • Build Your Patient Panel: Shape and manage your own panel, providing continuous and consistent care.
  • Collaborative Care Environment: Join a dedicated interdisciplinary team, including APPs and care coordinators.
  • Professional Growth: Benefit from ongoing education, clinical support, and involvement in quality improvement initiatives.
  • Balanced Practice: Flexible scheduling with manageable patient panel sizes designed for sustainable, high-quality care delivery.
