POSITION SUMMARY: Serves as ACT Lead Therapist for operations of regional Assertive Community Treatment 24/7 team, administrative and clinical supervision of support and professional staff, coordination with other administrative and clinical programs. Ensures consumer care and fidelity to evidence based practice. Responsible for coordination with community partners and managed care organizations.

QUALIFICATIONS: Licensed as a social worker, 4 years working with the SMI population.

SKILL SETS: Good computer skills, superior oral and written communication skills, articulate, trustworthy and decisive. Other skill sets include but are not limited to highly organized, good anticipatory planning, conflict resolution and analytical skills.

PHYSICAL AND ENVIORMENTAL DEMANDS: Must have visual acuity sufficient to read a variety of written reports and documents; hearing needed sufficient to understand spoken communications by phone and face-to-face with many staff, consumers, and external sources

JOB SPECIFIC FUNCTIONS:
  1. Participates in Interviews and recommends employee personnel actions (hiring, terminations, promotions, etc.).
  2. Develops/monitors orientation of new employees to program, department and agency policies and procedures.
  1. Reviews referrals and admissions, continued care and discharge based on level of care and medical necessity and ensures thorough aftercare plans and communication to/from referral sources and aftercare providers.
  2. Monitoring appropriate medical records documentation and billing practices
  3. Exercises sound clinical and ethical judgment in treatment decisions and responsibility.
  4. Coordinates interface with DDID, Regional Outpatient Clinics, ACSU, CSH, HMH and collateral partners.
  1. Collaboration with referral sources and community partners to educate on level of care.
  2. Provides leadership with multidisciplinary team in administrative and treatment staffing to assist in carrying out comprehensive treatment plan.
  3. Oversees level of care changes in coordination with managed care guidelines and assists in clinical review for preauthorization and continued stay reviews.
