Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not guaranteed Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase EMR and Oasis Job Description: - Provide, coordinate, and direct home nursing care according to physicians' orders, based on agency policies and procedures, through the competent application of the nursing process.- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.- Conduct home visits with an average of 5-8 patients per day.- Participate in periodic office meetings; reliable transportation, a current driver's license, and agency-required liability insurance are required.- Travel up to 75 miles from the home clinic; mileage reimbursed at IRS standard rate.- Types of cases include Start of Care, Wound Care, and Follow-up Visits.- Participate in on-call rotations as required, including potentially 1 weeknight and 1 weekend day every 6 weeks.- Charting and documentation proficiency required in HomeCare HomeBase and Oasis.- Execute visits primarily in patients' homes and cover designated territories.- Open to floating to nearby clinics if additional hours are needed due to low patient census.- Must have used HomeCare HomeBase EMR within the last 6 months or be willing to take a retraining course if used outside of the last 6 months.- Open to weekend shifts and Baylor Shift schedules, as applicable.
