Position: Director of Rehabilitation
Location: Sonoma Post Acute
Salary Range: $130k-$150k

- Lead and manage the rehabilitation department, including physical, occupational, and speech therapy services.
- Develop and implement strategies to improve patient outcomes, ensuring the highest standards of care.
- Collaborate with the interdisciplinary team to create individualized care plans that meet the unique needs of each resident.
- Oversee and mentor a team of therapists, fostering a culture of continuous learning and professional growth.
- Ensure compliance with all regulatory requirements and maintain accurate documentation.
- Monitor department performance, manage budgets, and optimize resources to achieve successful financial outcomes.
- Current California license as a Physical Therapist, Occupational Therapist, or Speech Language Pathologist.
- Proven experience in a leadership role within a skilled nursing or rehabilitation setting.
- Strong knowledge of skilled nursing and recovery processes.
- Excellent leadership and organizational skills, with the ability to inspire and manage a team.
- Passion for patient care and a commitment to making a positive impact.
- Strong communication and interpersonal skills, with the ability to work collaboratively across departments.
