Coordinates discharge plans and/or care plans for in/outpatients, including developing plans for ongoing treatment through services and agencies. Performs a psycho/social assessment for continuity of care planning for in/outpatients. Provides short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability. Participates as a member of an interdisciplinary team to develop a coordinated care plan for patients. Participates in quality assurance and utilization review to enhance patient satisfaction.
