Northern Counties Health Care is seeking a highly organized and detail-oriented Registered Nurse (RN), Licensed Practical Nurse (LPN), or Medical Assistant (MA) to join our Hardwick Area Health Center team. As a key member of our patient care team, you will work under the direction of the Practice Manager to deliver direct support to the clinic, colleagues, and providers, ensuring the smooth delivery of care and upholding our commitment to quality.
Responsibilities:
- Triage patient calls, utilizing your clinical knowledge to assess needs and prioritize care.
- Conduct thorough patient intake interviews and assessments, documenting accurate and up-to-date information in our Electronic Medical Records (EMR) system.
- Administer medications and vaccines, following established protocols and ensuring patient safety.
- Perform routine laboratory tests and phlebotomy, maintaining accurate records and adhering to quality standards.
- Assist providers with procedures and treatments, demonstrating technical proficiency and a commitment to patient comfort.
- Educate patients on treatment plans, medications, and health maintenance, empowering them to take an active role in their well-being.
- Graduate of an accredited RN, LPN, or MA program.
- Strong clinical assessment skills and knowledge of disease processes and treatments.
- Proficiency in computer skills, including EMR systems, email, and basic office software.
- Excellent communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
- Ability to work independently and as part of a team, demonstrating initiative and a strong work ethic.
