Work with other professionals to help develop and administer various health, wellness and nutrition care, and cultural programs for NORC participants. Help plan and conduct food service or nutritional programs to assist in the promotion of health and control of disease.Interview clients to assess needs. inform clients about food and nutrition health practices and programs; promote weekly health and wellness activities and events. Identify community resources that address food access and insecurity, healthy food production, distribution, and consumption. Use multidimensional psychosocial assessment health indicator survey tools and patient health questionnaires to assess presenting problems in older adults. Communicate with some Chinese immigrants to assess their case, understand their needs, and render assistance to clients seeking support, benefits, or food and health resources. Conduct necessary, timely follow-up communications. Assist clients regarding personal crisis matters. Use electronic database (e.g., STARSPeerplace) to document and maintain client and case information; manage client information confidentially and professionally in accordance with New York City Department for the Aging policies. Update program director on case status. Participate in case conferences and in team and staff meetings; answer phone calls and messages; greet, interview, and communicate empathetically with clients.
