We offer services ranging from comprehensive short-term rehabilitation and transitional care to Alzheimer's and dementia care. Day to day as a RN MDS Coordinator As a Full-Time RN MDS Coordinator at Welling Healthcare and Rehabilitation, you will step into a dynamic role where your day-to-day responsibilities will be both rewarding and impactful. Expect to conduct comprehensive assessments of residents, ensuring accurate and timely completion of MDS documentation. You will collaborate closely with interdisciplinary teams to develop individualized care plans, addressing each resident's unique needs. Regularly, you'll participate in team meetings, providing valuable insights and updates on care initiatives. Additionally, you will maintain compliance with state and federal regulations, contributing to the overall quality of care provided in our facility. Your schedule will be Monday through Friday, 8 AM to 5 PM, allowing for a balanced work-life rhythm as you engage in meaningful work within a supportive environment.
