The Social Worker is responsible to provide in-depth Individual/Family counseling, Crisis Intervention Counseling, and Training of staff/guardians including behavioral issue identification, child development, autism and how to re-integrate back into family environments. In addition, the Social Worker will provide a collaborative Psycho-Social history for each of the children which would include not only comprehensive information of previous family, medical, placement history before their placement at SFI, but history and services provided while at SFI and what is proposed for their future based on their respective needs. The Social Worker will be responsible for, but not limited to:

  • Responsible for ensuring that the medically related emotional and social needs of the clients are met and maintained on an individual basis in accordance with policies and procedures, and current federal, state and local standards, guidelines, and regulations.
  • Coordinates and processes incoming referrals: liaison with referral agencies; collaborates with referral sources and SFI Treatment Team to facilitate prospective admissions.
  • Evaluates each child to develop their Individual Service Plans (ISP) and Individual Educational Plans (IEP) on an initial, quarterly and annual basis.
  • Gathers information from Treatment Team for inclusion in ISP documentation
  • Maintains monthly data sheets showing progress towards goals established for the child.
  • Updates the ISP based on respective child needs, as progress or issues are identified through on-going observations and evaluations.
  • Coordinates and facilitates all ISP meetings for the client and Treatment Team members, including DSS/REM Case Managers and any involved family members
  • Develops Behavior Plans for clients on Psychotropic medications and/or based on other behavioral issues identified in their ISP.
  • Assesses each child on an on-going basis, assessing their progress, issues, provides recommendations to the Treatment Team and publishes documentation for client services in addition to ISP and Behavioral Plans to include monthly social worker notes and daily progress notes for contacts, visits, and/or current issues
  • Act as liaison with client’s Family/Referring Agency/Legal Guardian/REM Case Manager in the following areas:
  • Central Contact for DSS/REM Case Managers, Family and Legal representatives; maintains collateral contact with DSS, REM, and Legal representatives, updating them on any new client needs, issues, etc.
  • Provide clinical case management services for children with developmental delays
  • Provides community support for the children involving home visits, school visits, court hearings/reviews, and other community service providers.
  • Provides referrals to families in need of counseling, housing, food and other medical and emotional resources.
  • Coordinates receipt of required documentation for legal and medical services to meet each child’s needs in accordance with SFI, state and HIPPA policies.
  • Coordinates and facilitates client discharges, which may include potential adoptions, new foster home placement or return to biological family.
  • Provides supervised visitation support during regular business hours and at least one weekend per month; coordinates and supervises community activities to include publishing the children’s monthly social activity calendar.
  • Participates, as necessary due to staffing issues, in transporting children to their medical and legal appointments.
  • Required to enter and retrieve data using computer systems, systems applications, and other office equipment.
  • Attend mandatory in-service training and staff meetings as scheduled.
  • Work collaboratively with other members of the health care team in coordination of the clients care as evidenced by other department feedback.
EDUCATION and TRAINING
  • Master’s Degree in Social Work from an accredited School of Social Work
  • Current licensure in the State of Maryland will be required
EXPERIENCE:
  • A minimum of one year supervised social work experience in a health care setting, working directly with individuals.
  • Must possess a working knowledge of COMAR regulations and OSHA guidelines
