Position Overview

As an RN Ambulatory Care Coordinator, you will play a vital role in planning and prioritizing care for individuals and populations, implementing strategies to promote optimal health. You will utilize your nursing expertise and manage a diverse caseload, aiming to enhance outcomes for patients, families, and health systems through educational initiatives and preventive care.

  • Evaluate referrals to understand patients' and caregivers' needs, intensity of care required, insurance benefits, and available resources.
  • Create care plans and provide recommendations to primary care providers, specialists, and the healthcare team to ensure consistent management of care.
  • Assess educational needs of patients and families, implementing tailored teaching strategies and referrals as necessary.
  • Engage in communication and collaboration with patients, their caregivers, and healthcare providers to facilitate access to appropriate services, enhancing health in a cost-effective manner.
  • Analyze and measure patient care outcomes, interpreting data trends to inform group care strategies.
  • Maintain thorough documentation of patient data, care plans, interventions, and outcomes in accordance with departmental guidelines.

Qualifications

  • Required: Bachelor’s Degree in Nursing or a healthcare-related field (all departments except Spectrum Health Ludington); BSN to be obtained within 7 years for those starting on or after January 1, 2017 (Spectrum Health Ludington Hospital).
  • 2 years of relevant clinical nursing or case management/managed care experience is required; 5 years is preferred.
  • Current MI Registered Nurse (RN) license is mandatory.
  • Basic Life Support (BLS) certification from either AHA or ARC required within 90 days.
  • Certification as a Case Manager (CCM) is preferred upon hire.
