Pay Scale: $20.52 an hour at 37.5 hours a week
A. GENERAL RESPONSIBILITIES:
Maintain primary responsibility for overall case/program management for consumers placed in Desert Haven programming, including coordination, conduct and follow-up of all related case management activities.

B. SPECIFIC DUTIES (essential functions are highlighted in bold print):

  1. Conduct agency tours, providing information regarding programs and services, as well as entrance and exit criteria for prospective consumers and their representatives as necessary.
  2. Act as Interdisciplinary Team chairperson, as directed, in order to facilitate and conduct admission meetings, formal annual Individual Habilitation (Service) Plan reviews, informal reviews, discharge meetings, special-called meetings, etc.
  3. Provide person-centered case management services for consumers participating in the Project Independence DTAC Program, ensuring a collaborative, consumer-driven, consumer-focused process toward goal development, achievement, and service delivery.
  1. Conduct initial orientation for new consumers in accordance with established policy, ensuring coverage of all pertinent informational areas, distribution of designated materials and completion of appropriate paperwork.
  2. Complete consumer permission form with consumer to determine desire for parent/care-provider attendance at annual reviews and other ID Team meetings, as well as information to be shared (if any) with consumer representatives.
  3. Conduct initial evaluation of all new consumers and ongoing evaluation for continuing consumers.
  4. Collect, review and consolidate supervisor update review summaries, evaluation/assessment results, consumer input and behavioral and performance observations to formulate consumer IHSP evaluation summaries and goal development. Ensure IHSP's are developed in keeping with each consumer's hopes, dreams, and desired program outcome(s).
  5. Facilitate the referral process for consumers requiring programs and/or services beyond the scope of programs and services provided by DHE, documenting such on referral forms and tracking progress via the ISP process.
  6. Complete incident reports, accident reports, conference notes, IDT notes, suspension notices, Special Incident Reports, APS reports, etc. as needed and distribute to appropriate staff and/or agencies within established timeframes.
  7. Provide ongoing consumer case management and support, as needed.
  8. Conduct exit interviews with terminating/exiting consumers and prepare discharge summaries. Complete follow-up reports within 30 days and again within one year of discharge.
  1. Act as primary liaison between DHE and consumer's parents and care-providers, Regional Center counselors, APS officials, law enforcement officials, licensing representatives, etc. in consumer-related matters.
  2. Respond to consumer-related first aid and other related emergencies and a) determine need for EMS transport and ensure call is made as required; b) contact parents/care-providers and Regional Center, as appropriate, to advise of action taken by DHE and arrange for pickup, transport, etc.; c) follow through with consumer until situation is resolved; d) ensure completion of accident/incident report by appropriate staff (first on scene), review for accuracy and submit copies to designated staff, filing original within 24 hours in consumer case record.
  3. Assist in maintaining (including purging) and updating consumer case records, as required, in accordance with CARF standards and DHE policies and procedures and coordinate with consumers, parents/care-providers and Regional Center counselors to obtain missing or updated case record information.
  4. Assist in the coordination and/or facilitation of daily activities as directed.
  5. Complete interoffice communication and distribute to appropriate staff as necessary.
  6. Develop understanding of CARF standards and related DHE policies and procedures as such relate to assigned program area and related services.
  7. Ensure knowledge and understanding of Title 17 and Title 22 regulations, as applicable and as required, and related paperwork and responsibilities.
B. SPECIFIC DUTIES, (Essential functions are highlighted in bold print), Continued:
  1. Participate in DHE Speaker's Bureau within community as directed.
  2. Follow all safety rules, use good judgment regarding safe work behavior, cooperate with safety efforts and report all potential safety hazards immediately.
  3. Ensure compliance with consumer rules, regulations and guidelines as identified in the Consumer Handbook and advise supervisor of any discrepancies.
  4. Effectively interact with other departments as required in order to maintain a cooperative working environment and to better ensure consumer success.
  5. Advise direct supervisor of any and all communications and/or happenings that are significant and/or unusual.
  6. Serve as positive role model for staff and consumers through adherence to regulations set forth in the Desert Haven Personnel Handbook.
  7. Comply with DHE's Code of Ethics and other agency policies and procedures relative to business practices, interpersonal relationships and personal conduct and advise direct supervisor or H/R department of any discrepancies or infractions noted.
  8. Attend meetings, in-services, etc. as required.
  9. Perform other related duties as assigned.
QUALIFICATIONS:
  1. Minimum General Education: Bachelor's degree in social work or related field preferred. Five years experience in performing similar duties, with a counseling certification, may be substituted for degree.
  2. Specialized Education or Training: First Aid/CPR certification; One year supervisory and/or training experience with persons with disabilities; Familiarity with CARF standards and process preferred; understanding of counseling methodologies required.
  3. Minimum Work Experience: Minimum two years experience in performing similar duties, preferably with persons with developmental disabilities.
D. Special Skills or Attributes Required:

  3. Ability to be discrete in maintaining all confidential and restricted information.
