In this role you will be serving the community along with fulfilling our mission to provide culturally and linguistically congruent, comprehensive behavioral healthcare in a holistic manner, assuring access and quality to our diverse customers.

ESSENTIAL DUTIES AND RESPONSIBILTIES:

  • Assist in the assessment and intake process for women entering the MOMs program.
  • Establish a working agreement with the Provider to ensure the client achieves and maintain stabilization, housing and other related social problems.
  • Participate in community outreach events as requested.
  • Assist Providers in developing curriculum to meet clinical guidelines for the MOMS Program.
  • Conduct Government Performance and Results Act (GPRA) completion for clients entering the MOMS program and others as requested.
  • Participates in weekly case conference meetings and supervision to ensure effective interventions.
  • Develop community resources and referrals for healthcare, substance abuse treatment services, mental health services, childcare, food and clothing, domestic violence services, shelter, vocational/educational resources, parenting assistance, etc.
  • Participate in weekly calls with community providers as requested (ie. Weekly HOPE call).
  • Maintain community resources on-site in community resource binders.
  • Coordinate collateral contacts with all providers involved with the client.
  • Follow-up appropriate termination procedures.
  • Timely completion of documentation, client assessments and statistics for quarterly reports.
  • Meets regularly with the Supervisor to ensure achievement of the required minimum level of direct service productivity based upon defined productivity standards set by agency standards.
  • Maintains all certification standards required by funding sources, the State of Ohio and the agency; participates in educational and training opportunities to enhance professional knowledge and skills.
  • Other duties as assigned.

EDUCATION & EXPERIENCE:
  • High School Diploma Required.
  • Associate degree preferred in human services or related field or life experience that equates to an Associate Degree.
CERTIFICATES, LICENSES, REGISTRATIONS: Certified Chemical Dependency Assistant (CDCA).

LANGUAGE SKILLS: Ability to communicate with team members, leadership, and external contacts in a professional manner. Ability to collect specific data, complete necessary forms and prepare and present information in an understandable format. Ability to address questions related to the program. Ability to consult with other medical and social service providers.

REASONING ABILITY:
  • Ability to maintain data related to the program.
  • Ability to provide culturally competent care. Experience in providing crisis intervention, education, and case management, advocacy, and community resources.
  • Knowledge of substance use and mental health treatment issues/interventions.
  • Ability to develop trusting relationships with individuals at-risk for substance abuse.
  • Ability to consult with other staff and social service providers.
  • Ability to work cooperatively with other agencies/organizations to connect clients to community services.
  • Ability to facilitate parenting groups.

PHYSICAL DEMANDS: While performing the duties of this job, the employee is regularly required to talk, hear, stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls, and reach with hands and arms. The employee frequently lifts or moves up to 10 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.

SUMMARY: The Care Coordinator connects pregnant and post-partum women with opioid use disorder with services which may involve going to appointments with the client; helping the client achieve and maintain a successful, healthy lifestyle and independent living through support, advocacy, and coordination of services. The Care Coordinator assists the client in applying for social, medical, educational and entitlement programs; serves as an advocate and liaison for individuals/families and service providers; makes referrals to community agencies for housing, transportation, employment, personal safety, food, clothing, childcare, as well as other social services. Facilitates parenting groups for the clients.
