Responsibilities Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care. Completes OASIS assessments timely and accurately. Provides skilled nursing care to patients in accordance with the plan of care and agency policies. Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources. Initiates appropriate referrals when indicated. Promotes and maintains patient health through teaching and appropriate rehabilitative measures. Assists patients in learning appropriate self-care techniques. Other duties as assigned. Qualifications Current RN license, specific to the state(s) you are assigned to work. One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical. Current CPR certification. Valid driver's license, reliable transportation and liability insurance.
