VNS Health is seeking a Registered Nurse (RN) Clinic Care Manager for a nursing job in Cliffside Park, New Jersey.
Our clinical care management services are anchored in evidence-based practices to optimize the utilization of healthcare benefits and services. We are dedicated to delivering personalized care that empowers patients to safely manage their health within the comfort and security of their own homes and communities.
Responsibilities:
- Coordinate benefits and services for assigned members
- Complete care management and disease specific assessments
- Make timely telephonic care management calls based on risk level
- Resolve complex issues and member complaints impacting service delivery
- Provide health education to members and caregivers
- Assess social determinants of health and provide care coordination
- Identify member safety issues and intervene as necessary
- Advocate for members to obtain health care and other needed services
- Utilize care management process to set goals and implement interventions
- Promote adherence to physician treatment plans and provide education
- Educate, coordinate, and provide resources to reduce inappropriate utilization of emergency services
- Participate in interdisciplinary team meetings
- Ensure compliance with policies and regulations
Qualifications:
- Associate degree in nursing or bachelor's degree in physical therapy
- Minimum two years of RN experience
- Active RN license in New York
- Population Care Coordination Certification preferred
- Care Management, Case Management, OASIS, or other application certification preferred
