We are seeking an experienced MDS Coordinator to add to the growing team As an integral member of the healthcare team, the MDS Coordinator plays a pivotal role in ensuring accurate assessments, meticulous care planning, and adherence to regulatory standards. The MDS Coordinator will be responsible for various tasks aimed at enhancing the quality of care provided to residents.

Key Responsibilities:
  • Conducting comprehensive assessments, MDS, and formulating care plans as per assigned duties.
  • Initiating and supporting activities outlined in care plans.
  • Timely creation and distribution of monthly care plan calendars.
  • Maintaining and updating all care plans and assessments as per regulatory requirements.
  • Conducting regular monitoring and auditing of clinical records to ensure accuracy and timeliness.
  • Alerting the Director of Nursing (DON) about persistent issues related to non-compliant documentation.
  • Upholding the strictest confidentiality regarding resident and facility information at all times.
Requirements:
  • Prior Experience: Demonstrated experience with MDS 3.0
  • Licensure: Valid RN License.
  • Professional Background: Previous experience in a long-term care setting
  • Organizational Skills: Must exhibit a high level of organization, professionalism, and motivation.
  • Technical Proficiency: Solid computer skills are essential for efficient documentation and data management.
  • Communication: Excellent verbal and written communication skills are necessary for effective collaboration with interdisciplinary teams and residents.
  • Team Player: Should possess a friendly demeanor and work well within a team-oriented environment.
