We are searching for a LPN - Home Care Liaison Coordinator to perform diversified duties including but not limited perform assessments of patients being referred to Home Care Services by the Case Coordinator; evaluate skilled services for home care within framework of health insurance reimbursement guidelines; investigate availability of resources and provide needed care, etc. Assist with education orientation of staff in regard to discharge planning and acceptance of patient issues as appropriate. Participate in all transition care initiatives to facilitate a safe discharge plan and prevent readmissions. May provide functional or technical supervision of other assigned personnel where scope of supervisory responsibility is limited to occasional direction, guidance, explanation, etc.

$3500 new hire sign-on bonus for this position. (rehires termed more than 1 year also eligible)

  • Hours: Per Diem
  • Shift: Day Shift 8am-4:30pm with weekend and holiday rotation
  • Location: St. Luke's Hospital, New Bedford MA

Education and/or Experience Requirements:

  • Graduation from an accredited school of practical nursing or equivalent is required.
  • Current LPN Licensure in the Commonwealth of Massachusetts is required.
  • A minimum of five (5) years related work experience is required.
  • A minimum of three (3) years current nursing experience in home health care is preferred.
  • In accepting employment with Southcoast, you understand that you are required to be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
