Beacon Medical Group (BMG) is seeking a dedicated and experienced Nurse Practitioner to join our hospital-based team. This position offers an exciting opportunity to deliver primary care and coordinate transitional care for patients preparing to transfer to rehabilitation, skilled nursing facilities, or home settings. The role also includes inpatient coverage in the rehabilitation unit and collaboration with multidisciplinary teams to provide outstanding patient care.

Key Responsibilities

  1. Clinical Care:
    • Deliver primary and nursing care under the direction of the Practice Manager/Director and in collaboration with the Practice Site Physician or designee.
    • Conduct patient rounds, perform consultations, and oversee transitional care for patients moving to rehabilitation or other care settings.
    • Obtain and document complete medical histories, perform physical exams, and interpret diagnostic data.
    • Develop and implement diagnostic and therapeutic plans, including ordering appropriate laboratory and diagnostic tests.
    • Administer medications, sutures minor lacerations, and manage medical and surgical emergencies when required.
    • Provide health education and counseling to patients and families, including preventative care guidance.
  2. Administrative Support:
    • Maintain accurate and timely patient records, charts, and other documentation.
    • Participate in interdisciplinary care conferences and assist with referrals to ensure continuity of care.
    • Assist in the development and monitoring of clinical protocols, policies, and procedures.
    • Contribute to the improvement of quality metrics and patient outcomes through participation in performance improvement initiatives.
  3. Education & Training:
    • Collaborate with and mentor nursing students, medical residents, and other pre-professionals.
    • Participate in ongoing educational and follow-up programs to support professional development and improve patient care.

Qualifications
  • Education: Bachelor's degree in Nursing required; Master of Science in Nursing preferred.
  • Licensure: Valid and current RN license in Indiana and national certification as a Nurse Practitioner.
  • Experience: Minimum of 2 years of clinical nursing experience; completion of a six-month Nurse Practitioner internship for new graduates or equivalent experience.
  • Certifications: Current BCLS (CPR) certification; prescriptive authority or ability to obtain within six months of hire.
  • Skills:
    • Strong clinical and analytical skills.
    • Effective interpersonal and communication abilities, both oral and written.
    • Proficiency in care planning and patient education.
  • Additional Qualifications: Bilingual skills (speaking/writing) are a plus.
Working Conditions
  • Primarily hospital-based with some time spent in inpatient rehabilitation units.
  • Requires physical stamina to walk/stand for prolonged periods, lift/position patients up to 35 pounds without assistance, and provide CPR when needed.
  • Exposure to biohazards and physical demands typical of a clinical environment.
