PURPOSE:

Functions as part of a therapeutic team providing support in a rehabilitative program to individuals residing in a Transitional Living Program . This position, Independent Living (IL) Program Case Manager, will assist with maintaining a therapeutic environment that promotes, healing, wellness, and hope. Provides guidance and direction to individuals to aid in successfully assimilating into the program as well as the ability to coach, model, and support individuals toward the attainment of communication, relationship, and interpersonal skills.

ESSENTIAL FUNCTIONS-JOB DUTIES:

  • Exemplifies an attitude and behavior that models the organization's values to staff, program participants, service providers and the community-at-large.
  1. Program Staffing
The IL Program Case Manager will participate in the communication and coordination process of the programs.
  • Review facility logs, discuss operational issues with staff, or supervise program activities.
  • Supervision of staff during routine daily activities.
  • Review non-case specific e-mail or other correspondence.
  • Review and completion of communication/shift change logs.
  • Mandatory attendance at all staff meetings of the Adolescent Program
  1. Maintain physical site:

The IL Program Case Manager will be responsible to ensure the maintenance and upkeep of the components of the Adolescent Programs.
  • Review facility logs, discuss operational issues with staff, or supervise program activities.
  • Completing reports and maintenance requests on paper and submitting it to the Health and Safety Staff or Adolescent Program Specialist.
  1. Reporting:

The IL Program Case Manager will submit weekly email reports to the Director of IL Programs and the Program Specialists indicating resident concerns, property concerns, and the schedule of appointments and visits for the following week.
  1. Fiscal:

The IL Program Case Manager will complete fiscal forms as necessary.
  • Completing time sheets
  • Completing expense reports
  • Requesting vouchers from Counties
  • Maintain receipts for vouchers and provide to the Director of Adolescent Programs for reimbursement by county.
  1. Compliance:

The IL Program Case Manager is responsible to implement all case management requirements daily.
  • Individual or group meetings to cover licensing requirements, the certification process and other preliminary steps prior to the filing of application forms.
  • Paperwork related to the activities of recruitment/licensing/recertification
  • Preparing and maintaining case record documents;
  • Implementation of case management requirements;
  1. Training:

The IL Program Case Manager will complete and maintain all training requirements in a timely manner.
  • Supervision
  • Meetings or Conferences regarding supervision of case; case/administrative reviews, Case Conferences, Team, Permanency planning Meetings;
  • Crisis Counseling/Intervention
  • Training - time engaged in or preparing for training, either as a trainer of other employees, or as a trainee
  • Supervising administration of medication
  1. Site Coverage/On-Call Crisis Support:

Adolescent Program Case Manager shall also assist and troubleshoot crisis situations with the program supervisors and assist in reporting crisis situations to placement providers; this will include participating in the "On-call Support" rotation.
  • Monitor all security, communications, and fire/life systems via the provided monitoring system
  1. Program needs:

The IL Program Case Manager will ensure the care and well-being of each youth at the components of the Adolescent Programs., daily evaluate staff interactions with youth, mentoring, encouraging, and correcting staff so that a positive environment is maintained.
  • Routine daily supervision of youth;
  • Supervision of youth during the course of the school day or during the course of extra-curricular activities;
  • Administering medications;
  • Routine administration of first aid to the youth;
  • Maintenance activities related to the facility;
  • Documentation: Progress Notes/paperwork pertaining to child's/youth's daily activities, including incident reports
  • Identifying appropriate placement resources, providing information to the placement resource, and arranging for placement of the child.
  • Providing the child(ren) with requested information about needed services, directing an individual to needed treatment, aid or information.
  • Preparing reports to the court or participating in a judicial proceeding.
  • Travel/transport of the child(ren) to/from changes in placement, case reviews, court hearings, appointments such as medical or counseling, works as well as any transportation related to activities identified within this code
  • Discussions with parents, relatives, foster parents, or others regarding the child or youth's environment and the need for placement or re-placement (or program transfer);
  • Referrals and follow-up to family planning services, parenting classes, job training, etc;
  • Reviewing the history of abuse, neglect, or juvenile delinquency to assess case plan;
  • Identifying the safety and well-being needs of the youth and his/her family for case planning purposes (this includes Safety Plans);
  • Contact with mental health, education or other professionals involved with the child/child's family to obtain information related to the child's psychological, developmental, behavioral and educational needs and goals;
  • Development of goals, service plans, written service agreements;
  • Routine contacts, other monitoring and/or communication with parents (biological, foster, adoptive) regarding the child in relation to the status of the child, the case plan, goals for the child and family, and administrative procedures of the agency;
  • Arrangement for and monitoring the provision of services identified in case plans;
  • Interviewing the child or youth as part of case management/supervision;
  • Participation in any court appearance related to the status of a child who is in placement;
  • Preparation of reports and testimony for judicial proceedings, and the participation in such proceedings
  • Direct administration of drug test for residents(s)
  • Crisis Counseling/Intervention
  • Scheduling Appointments as required by the OCYF 3800 Regulations.
  • Create Transportation Schedule as needed
  • Lias between Centerville Clinics and the Adolescent Programs
  • Request any needed information from the County/Probation Officer
  • Close all resident files on the fifth day after discharge
  • Ensure all new resident binders are complete and neat
  • Update all residents in PrecisionCare
  • Monitor/check each apartment two times per shift.
  • Other duties as assigned by the Director of Adolescent Programs or the Adolescent Program Specialists.
JOB REQUIREMENTS:
  • EDUCATION/KNOWLEDGE REQUIREMENT: Specific Education, Certification, and/or Licensure:
3 years working in a case management or related field where attention to detail is a focus.

CPR/1st Aid Certification (company will provide training)
  • ESSENTIAL JOB RELATED EXPERIENCE:
Superior level knowledge of OCYF 3800 Regulations;
  • PREFERRED JOB RELATED EXPERIENCE:
    Five (5) years of experience working with youth in a residential settings
    Microsoft 365 Suite
    Basic Computer Knowledge
Bachelor's Degree in Human Services Field
  • ORIENTATION:
    Complete both online and in person training
  • YEARLY TRAINING:
    40 hours mandated by OCYF
    10 hours of leadership training
    10 hours supervision/teamwork training
    Other trainings as assigned by Executive Director
  • ADDITIONAL MANDATORY ANNUAL TRAINING:
    PA Mandated and Permissive Reporting
    Becoming Trauma Aware
    Project Starfish
    Safe Crisis Management

  • CLEARANCES:

  • ADDITIONAL REQUIREMENTS
    Position requires the ability to work a flexible schedule
    This position will work one weekend (Saturday and Sunday) each month

Supervisors will have access to all records in the CRR programs for on-call purposes only.
