Job Title: MDS Coordinator

Job Specialty: Nursing

Guaranteed Hours: 40 hours per week

Experience: Minimum 2 years required

License: Current licensure by the State Board of Nursing as an RN

Certifications: BLS, ACLS, MDS-Certified preferred

Must-Have: MDS experience, ability to work the floor if staffing demands, knowledge of RN duties

Job Description:

- Assess and coordinate the assessment of all residents
- Complete the Resident Assessment Instrument (RAI) and transmit RAI data electronically
- Transfer Resident Assessment Group Data (RUG) to PatCom system
- Facilitate communication between the Interdisciplinary Team
- Evaluate patient care outcomes, administer medication, and manage care
- Provide direct patient care, including bathing, dressing, and feeding patients, and assisting in examinations and treatments
- Ensure physical capability to lift, walk, carry, and stand
- Schedule completion of RAI following guidelines and input data into the RAI computer system
- Supervise completion of the RAI by the Interdisciplinary Team and ensure accuracy
- Transmit RAI data weekly
- Enter weekly submission RUG score into PatCom system and coordinate billing
- Train new personnel in RAI completion and provide education updates to current staff
- Implement resident care plans and coordinate weekly Interdisciplinary Team care plan conferences
- Monitor the RAI process and develop improvement plans
- Maintain clinical competency to provide direct care as needed
- Maintain professional conduct and utilize critical thinking for problem resolution
- Attend annual in-services and mandatory staff meetings
- Possess good organizational, leadership, oral, and written communication skills
- Deal effectively with the needs of the geriatric population

Additional Information: Candidates must meet specific requirements and possess a positive attitude towards residents, staff, and families. Certification in geriatrics or MDS is desirable. Experience with RAI and in Long Term Care is preferred.
