         STATEMENT OF WORK:

The contract pharmacist shall provide appropriate medical evaluation and/or treatment of patients who present for care in the IHS facility. The contract pharmacist shall provide professional advice to appropriate clinic and/or ward personnel in admission situations with regard to professional management of patients being treated and/or hospitalized.

Pharmacist will provide patient services initially with counseling and basic needs of the patient care; management and daily operation of the outpatient pharmacy, processing EHR scripts, chronic medication refills, and counseling patient on proper use, precaution and storage of medication. Pharmacist will be performing a combination of duties in the pharmacy department such as patient education, counseling and provide assistance with patient monitoring.

 

Dispense medications prescribed by a physician. Informing doctors about a patient's medication history, monitoring treatment programs, administering drug therapies, and providing instructions on the correct usage of prescription medicine. Dispensing and monitoring a patient's medication, and protect patient privacy. Answer incoming phone calls regarding specific question pertaining to refills and new scripts.

Counseling patients about health concerns, such as diet, exercise, stress management, and home health care supplies. Assist patients with questions, such as possible side effects and proper dosages for over-the-counter and prescription medications. Provide information on topics, such as how to quit smoking, or give advice for managing chronic conditions, like diabetes and asthma.

Verify prescription orders that are unclear or potentially harmful for a patient, confirm the dosage and formulation (e.g., liquid or tablet), as well as whether brand name product is required or if you can substitute a generic equivalent. Report all drug adverse events.

Establish and maintain good relationships with physician/healthcare practices. Assist in streamlining communication with the patient, insurance companies, nurses, lab and other healthcare-related parties.

Check each patients’ medication record every time he or she gets a new or refill prescription filled. This is the best way for a pharmacist to prevent potentially dangerous interactions between drugs.

Maintaining patient’s health requirements when taking prescription medications, help patients heal and avoid getting sick by sharing advice on using nonprescription remedies, taking health supplements such as vitamins, using herbal and natural health products, exercising and maintaining a good diet.

Maintains accurate records of patient workload on a daily basis via RPMS and the Electronic Health Record.

    1. KNOWLEDGE REQUIRED BY THE POSITION

Knowledge of the physical/chemical properties/characteristics of pharmaceutical substances; therapeutic action of drugs; and principles of chemical, biological, pharmacological, and biopharmaceutical science.

      1. Reviewing and verifying appropriateness of prescriptions
      2. Reviewing patient medical records for allergies
      3. Filling prescriptions
      4. Physically checking prescriptions filled by authorized medical staff, including the pharmacy technician and prescribing clinicians, and documenting the verification beside the medical record entry (prescription)
      5. Ensuring local and national formularies are utilized by the clinicians
      6. Maintaining adequate inventory of medications
      7. Providing information to the Pharmacy and Therapeutics Committee on a quarterly basis or as needed
      8. Serving as a resource for the clinicians regarding pharmacological issues/concerns
      9. Providing patient education to the inmate population
      10. Completing administrative duties such as adverse drug reaction reporting, notifying the Clinical Director or their designee of discrepancies in prescriptions
      11. Conducting pill lines as needed.services are a contractual arrangement and not a personnel appointment. The services do not constitute an employee/employer relationship. Payment is based on the provision of an end product or the accomplishment of a specific result. The contractor will not be subject to government supervision, except for security related matters. However, contractor performance shall be closely monitored.

Schedule
 

  1. function of this position is to provide a wide range of pharmacy services that will provide pharmacist coverage in the absence of the Chief Pharmacist I Staff Pharmacist during annual leave, extended medical/sick leave, and continued professional education. se services will be performed on an as needed basis, as determined by the health services department at AUSP Thomson. Work will be performed between the hours of 6:00 am and 2:30 pm, Monday through Friday (except Federal holidays), normally consisting of six (6), one (1) hour sessions per day, with a 30 minute unpaid lunch period, (if applicable). facility's need will vary based on leave and training requirements/requests of the Chief Pharmacist I Staff Pharmacist, but is expected to require an average of 10-40 sessions per month. Contracting Officer Representative (COR), based on communication with the Chief Pharmacist, will be responsible for defining each weekly schedule. schedule is subject to change based on the variable needs of the institution and the unpredictable nature of the correctional ironment.Upon agreement, any changes to the schedule must be coordinated with the COR assigned. However, the daily "call-out" will dictate the exact schedule and sessions required. A mutually agreed upon schedule will be determined following award. Contractor may be subject to schedule changes and must be flexible. Contractor will be available for emergencies as necessary.
