MUST:

Experienced Social Worker

Bachelor or Master's Degree in Social Work (M.S.W.) from an accredited college or university or other health related degree.

Practicum or professional experience working with geriatric population preferred.

Administrative ability and computer proficiency required.

CCRC experience preferred.

DUTIES:

Maintains a case load of skilled nursing, long term care and assisted living residents

Works as part of a multidisciplinary health care team which provides services to residents

Completes screening and intake assessment of all new admissions to the Center to assess biological, psychological, social and spiritual needs. Completes a psychosocial history assessment upon admission and periodically with accordance to state and federal regulation.

Develops a resident-centered preliminary and comprehensive plan of care for each resident which identifies the problems / needs for the resident and the goals to be accomplished for each problem / need.

Reviews code status with each new admission.

Uses applied theory to identify and implement resident-centered therapeutic interventions.

May provide, as needed, short and long term counseling to residents and their families under the supervision of a Licensed Clinical Social Worker.
