POSITION SUMMARY

The Hospital at Home Paramedic performs patient care under the direction of and as assigned by the charge nurse, registered nurse and/or physician or their designee. Performance in this care model utilizes continuous communication within the medical care team, either by radio/phone contact or direct consultation. The Paramedic delivers appropriate direct and indirect patient care in compliance with regulatory and department processes while promoting safety and efficiency within the home.

PRIMARY ACCOUNTABILITIES:

  • Initiates/completes technical equipment set-up for remote monitoring to assist RN in timely admissions. Detects changes in assessment and notifies medical care team of abnormalities. Initiates oxygen therapy set-up and/or application, to include nebulizer treatments, as directed by RN/MD.
  • Inserts/discharges peripheral IV’s; administers IV fluids, oral and parenteral medications as directed by RN; including, but not limited to, observing and documenting condition of infusion site, medication delivery, and antibiotic administration (scheduled).
  • Performs venipuncture for specimen collection and delivers to testing facility.
  • Receives unmonitored patients at registered facility (campus) for diagnostic testing/procedure completion; prepares and transitions patient for transport home once admission has begun and destination address confirmed.
  • Assists physician or RN with procedures and exams, applies/changes dressings/slings as directed. Follows CDC isolation guidelines, appropriately uses personal protective equipment (PPE), performs hand washing according to policy, and enforces safety regulations.
  • Serves as a customer service champion proactively acknowledging and engaging patient/patient family/support involvement in care.
  • Maintains work area cleanliness, cleans remote monitoring device kits for distribution, tests equipment prior to admission, and collaborates with device kit vendor to ensure kit maintenance.
  • Is proactively involved in patient care planning and task completion to support care team and patient; available as directed for continued care needs including deliveries of supplies, meals, and/or medications.
