JOB DESCRIPTION:

A Home Health RN provides comprehensive nursing care to patients in their homes. They assess patients' health, develop care plans, and administer treatments while collaborating with other healthcare professionals to ensure patients receive the highest quality care.

KEY RESPONSIBILITIES:
• Patient Assessment: Conduct thorough assessments of patients' medical conditions, including physical, emotional, and social factors. Gather information on their medical history, current medications, and any existing health issues.
• Care Planning: Develop individualized care plans tailored to each patient's needs. Collaborate with the patient, their family, and the healthcare team to set goals and establish a plan of care.
• Medication Management: Administer medications, injections, and intravenous therapies as prescribed. Monitor medication adherence and educate patients and families on proper medication management.
• Wound Care: Perform wound care, dressings, and other procedures as needed, ensuring that wounds heal properly and are free from infection.
• Disease Management: Provide education on managing chronic conditions, such as diabetes, hypertension, and heart disease. Monitor patients' progress and adjust care plans as necessary.
• Monitoring and Vital Signs: Regularly monitor patients' vital signs, including blood pressure, heart rate, respiratory rate, and oxygen saturation. Detect and respond to any changes promptly.
• Teaching and Education: Educate patients and their families on managing their conditions, maintaining a healthy lifestyle, and recognizing warning signs of deterioration.
• Assistive Devices: Train patients in the proper use of medical equipment or assistive devices, such as ventilators, oxygen therapy, and mobility aids.
• Safety and Infection Control: Ensure a safe and clean home environment for patients. Educate patients and caregivers on infection control practices.
• Documentation: Maintain accurate and detailed electronic medical records of all patient assessments, care provided, and changes in condition. Ensure compliance with regulatory requirements.

SUBMISSION REQUIREMENTS:
• 1 year of home Heath experience - REQUIRED
• Graduate of an accredited school of nursing with 1 year of recent acute care experience - REQUIRED.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order - REQUIRED.
• Good typing skills - REQUIRED.
• Wound care, wound vac, catheter, acute care experience - REQUIRED.
• Doesn't need CA license at time of submittal but will have to be willing and able to obtain.
