Clinical Pharmacist

Job Summary
• Reports to Manager, Pharmacy Services.
• Compounds and dispenses medication pursuant to a physician order.
• Monitors drug therapy for appropriateness of drug, dose, route, and duration of therapy.
• Screens for drug interactions, incompatibilities and contraindications.
• Recognizes potential adverse drug effects and intervenes as appropriate.
• Provides drug information to physicians, nurses, allied health professionals, and patients.
• Participates in interdisciplinary team conferences and other patient-care activities.
• Cares for all age group patients (birth and above).
Job Requirements
Minimum Education
• B.S. or Pharm. D. degree from an accredited College of Pharmacy.
• Must provide evidence of meeting state mandated continuing education requirements upon request.
Minimum Work Experience
• Previous hospital pharmacy experience is highly desirable.
• Previous pharmacy computer experience desirable.
Required Licenses
• [Kentucky, United States] Pharmacists
• Licensed as a Registered Pharmacist by the Kentucky State Board of Pharmacy (or must be Kentucky licensed within 90 days of employment).

ESSENTIAL FUNCTIONS
Responsible for accurate dispensing of medications.
• • Reviews and interprets physician orders properly; screens for incompatibilities, interactions, contraindications.
• • Contacts nursing personnel and/or prescriber regarding any questions on drug orders.
• • Accurately fills drug orders using triple-check method.
• • Accurately generates labels using proper format for dispensing not covered within the pharmacy computer system.
• • Accurately enters patient orders onto computerized profile; questions and rectifies any duplication of therapy.
• • Accurately prepares IV solutions, hyper-alimentations, epidural fluids and other sterile products using aseptic technique.
• • Accurately prepares chemotherapeutic agents in vertical-flow hood using appropriate handling techniques.
• • Processes patient charges accurately.
• • Accurately checks all non-Pyxis items prior to delivery (IV and Med orders)
• • Accurately checks all stock supplies prepared by technicians.
Exercises controls on controlled substances maintained/dispensed.
• • Maintains physical inventory and computerized records.
• • Properly labels floor stock narcotics and matches to dispensing records.
• • Accurately compounds epidural infusions or other narcotic-containing products.
Effectively participates in clinical services provided by department.
• • Able to adequately perform drug usage evaluations as assigned.
• • Able to effectively participate in patient care conferences and other assigned committees and other patient care roles.
• • Provides pharmacokinetic dosing and drug consults/monitoring as requested.
• • Provide accurate, clinically-relevant drug information to physicians, nurses, allied health professionals, and patients as needed.
Meets administrative and miscellaneous requirements of department.
• • Participates in departmental QAI programs.
• • Reports workload statistics to manager as requested.
• • Supervises nonprofessional personnel and organizes/directs their workflow as needed.
• • Meets departmental cost initiatives (formulary compliance)
• • Demonstrates a consistent level of performance.
• • Demonstrates good professional judgment and competence in performing professional functions
Performs miscellaneous duties and follows departmental guidelines.
• • Communicates information effectively, within the department and the hospital.
• • Serves as an active member on hospital committees as assigned by the pharmacy manager.
• • Assists in training of new employees and pharmacy students.
• • Participated in hospital and community drug education programs as assigned.
• • Projects a professional image for the department.
• • Demonstrates an understanding of basic hospital and departmental policies.
