Job Description

  • Coordinate and participate in team meetings/huddles and delegate tasks to appropriate team members
  • Identify clinical needs proactively by reviewing and monitoring monthly quality reports
  • Collaborate in developing team priorities and patient goals
  • Assist in patient education, goal setting, self-management, and coaching
  • Manage referrals and answer calls in real time
  • Assist with coordination of staffing schedules and ordering supplies
  • Assume responsibility for overall clinic appearance and cleanliness
  • Routinely evaluate clinic for quality compliance
  • Act as a preceptor to new staff and attend coordinator meetings
  • Follow through on organizational operations

Qualifications
KNOWLEDGE/EXPERIENCE: 2 years clinic experience preferred. Experience in overseeing unit operations preferred.

EDUCATION: Current license as a Licensed Practical Nurse.

LICENSE/CERTIFICATION/REGISTRY: Current state Board of Nursing licensure as LPN. Basic Life Support (CPR) certification and re-certification every two years.
