Position Summary Analyst, Case Manager is responsible for telephonic assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies.

Required Qualifications

  • New York State Identification Card

  • Bi-lingual Cantonese/Mandarin

  • 2 years experience in behavioral health, social services or appropriate related field equivalent to program focus

Preferred Qualifications

  • Case management and discharge planning experience preferred

  • Managed Care experience preferred

  • MLTC experience preferred

Education

  • Social Worker with Bachelor's degree or non-licensed master level clinician required

  • Degree being in behavioral health or human services pref erred (social work, psychology)

Pay Range

The typical pay range for this role is:

$19.52 - $47.03

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
