We have an opening for a:
Part-time RN Care Manager
(TEMPORARY)

The Part-time (Temporary up to 1 year) Registered Nurse Care Manager licensed in the state of Wyoming or part of the Nursing Licensure Compact Agreement (NLC).  As a member of the team, you would be responsible for providing case management services to high acuity clients with complex physical, social, and behavioral health conditions. You will develop plans of care, outline client goals and planned interventions, connect clients to community resources, coordinate medical appointments and provide health education.

This position is a Part-time (less than 20 hours a week), Temporary position, however, candidates must reside in Wyoming or a bordering state. All duties will be performed over the phone and using various computer applications. Exceptional customer service, oral communication and behavioral health experience is preferred. Targeted compensation for this position is $28.04 - $36.05 an hour. First review of candidates will be on August 26th, and continually reviewed every 5 days after.
