Position Objective: Join our compassionate team as a Social Worker - Clinical Specialist, where you will provide essential counseling, crisis intervention, and transition planning services to patients and families. Your expertise will be invaluable in performing psychosocial assessments, planning, and implementing services to support patients at all ages through their healthcare journey.

Essential Job Duties:

  1. Conduct comprehensive psychosocial assessments for patients identified by Care Coordinators or the interdisciplinary team, in line with established guidelines.
  2. Create a therapeutic environment to promote effective problem-solving for patients and their families regarding illness adjustments, long-term care, end-of-life issues, and other mental health concerns.
  3. Facilitate complex discharge planning, addressing challenges like substance abuse, psychiatric services, and guardianship needs.
  4. Identify community services and resources, ensuring strong connections for patients and families.
  5. Collaborate proactively with the interdisciplinary team, sharing updates and recommendations to enhance patient care.
  6. Evaluate patient and family progress towards established goals, organizing care conferences and support groups as necessary.
  7. Integrate relevant medical record and nursing information to inform assessments and interventions.
  8. Ensure timely and accurate reporting of any suspected child or elder abuse, incorporating domestic violence resources as needed.
  9. Maintain clear documentation of interventions in the patient's medical record, following departmental standards.
  10. Engage in process improvement initiatives within the care management department, identifying areas for enhancement and mentoring new staff and interns.

Educational/Experience Requirements:

  • Master's of Social Work
  • Minimum of two years' experience in a healthcare delivery system, ideally in acute care settings.

Required License/Certifications:

Active Licensed Clinical Social Worker (LCSW-C) certification from the Maryland Board of Social Work Examiners.

Working Conditions, Equipment, Physical Demands:

Be prepared to face possible exposure to blood-borne pathogens in this role.

Physical Demands: This role entails medium work, where you might need to exert up to 50 pounds of force occasionally, 30 pounds frequently, and 10 pounds continuously to move objects.
