RESUMEN DEL PUESTO:

Brinda servicios integrados directos de salud mental en apoyo a la atención primaria integrada y los servicios de salud mental. La salud mental abarca la prevención, la intervención y la recuperación de las afecciones de salud mental y uso de sustancias dentro del entorno de atención primaria en Chase Brexton Health Care.

PRINCIPALES FUNCIONES Y RESPONSABILIDADES:

Pensamiento analítico y crítico

  • Realiza exámenes breves, basados en pruebas y apropiados para el desarrollo.
  • Realiza evaluaciones en tiempo limitado.
  • Aplica el conocimiento de la práctica basada en pruebas, que incluyen bases empíricas de evaluación, intervención y otras aplicaciones clínicas, experiencia y preferencias del paciente para tratar a individuos, parejas, grupos y familias que incluyen adultos, niños y adolescentes según lo asignado.
  • Brinda evaluaciones de emergencia a los pacientes en crisis y facilita la internación psiquiátrica si es necesario.
  • Evalúa la preparación para el alta a lo largo del curso del tratamiento.
Resolución de problemas y toma de decisiones
  • Crea e implementa planes de tratamiento integrados, garantizando el acceso a los servicios organizativos y externos dentro de los plazos asignados.
  • Evalúa de forma independiente el progreso del tratamiento y modifica la planificación según lo indicado, incluso en ausencia de medidas establecidas por resultado.
Comunicación
  • Brinda consultas a proveedores médicos, al personal de CBHC y a otros proveedores de servicios para facilitar la atención continua y coordinada dentro del entorno de atención primaria.
  • Participa en reuniones de personal según sea necesario.
  • Sus comunicaciones verbales, no verbales y escritas son informativas y están bien integradas.
  • Participa en el cambio del sistema y promueve el cambio dentro de Chase Brexton Health Care.
  • Asegura que las notas de progreso sean accesibles para el equipo de atención del paciente y considera que pueden ser accesibles para el paciente.
Enfoque en el paciente
  • Brinda referencias para servicios internos y externos.
  • Demuestra capacidad para involucrar a los pacientes o familias que no participan en los servicios a través de la divulgación y el seguimiento.
Cumplimiento, política y procedimiento
  • Documenta todos los servicios de salud mental según lo indique la política de CBHC, los requisitos estatales y los estándares profesionales.
  • Se adhiere a los estándares, políticas y procedimientos de productividad departamentales.
  • Si brinda supervisión: Demuestra conocimiento de los modelos y prácticas de supervisión; demuestra conocimiento y aborda eficazmente los límites de competencia para supervisar.
  • Evalúa el riesgo utilizando una medida estandarizada (por ejemplo, CAMS; C-SSRS).
  • Completa todas las tareas relacionadas con el cumplimiento de reglas y estándares.
  • Demuestra el uso apropiado de las órdenes de facturación para toda la documentación clínica.

  • Utiliza el registro electrónico de salud para apoyar y mejorar la atención médica integrada.
Ganas de aprender
  • Cumple con los requisitos obligatorios de educación continua para mantener su licencia/certificación.
  • Desempeña otras funciones asignadas por el supervisor.
COMPETENCIAS Y HABILIDADES
  • Debe tener la capacidad de brindar servicios que sean relevantes para la cultura del paciente y su familia. Debe ser capaz de identificar y abordar las disparidades en el acceso y la calidad de la atención médica.
  • Debe ser capaz de adaptar los servicios a las diversas normas culturales y promover la diversidad entre los proveedores que trabajan en equipos interprofesionales.
  • Debe estar familiarizado con el modelo de acceso abierto a la atención en entornos comunitarios de salud mental y atención médica. Debe poder adaptarse a los cambios en la atención médica a nivel estatal y nacional.
FORMACIÓN Y EXPERIENCIA:
  • Título requerido: Título de grado en Trabajo Social, Terapia, Doctorado en Psicología o PhD.
  • Preferidos: LCSW-C, LCPC, psicólogo.
  • Se requiere licencia: Licencia en el estado de Maryland.
  • Se dará preferencia a los candidatos con: Dos años de experiencia en la prestación de servicios ambulatorios de salud mental con adultos, niños o adolescentes. Serán considerados especialmente los candidatos con experiencia en un entorno de atención primaria.
