Job Summary:

JOB SUMMARY

The Public Outreach Specialist is responsible for serving as an important bridge between patients, families and VIPMS, facilitating the delivery of quality healthcare. Services as a neutral intermediary encouraging open communication and promoting fairness to enhance the patient experience and strengthen organizational commitment to service excellence. Is responsible for supporting and maintaining population health programs and care coordination activities. These activities are performed in a variety of settings including during outpatient clinic visits, emergency room visits and telephonically. Makes recommendations for process or service improvement opportunities while maintaining the privacy and confidentiality of all parties involved.

KEY RESPONSIBILITIES

• Supports population health program initiatives through outreach activities (calls to schedule needed services, reminder calls, etc.).

• Supports population health and disease management programs, including scheduling, data entry, report generation and distribution, and communication with internal and external entities

• Engages in interventions to reduce emergency room utilization and hospital readmissions by performing educational outreach to patients and families

• Assists patient with scheduling, follow-up and accessing healthcare benefits and resources

• Maintains accurate documentation in the medical record, including accurate demographics when contacting patients/caregivers

• Research patient eligibility and assignment status with regards to Medicaid managed care health plans to ensure patient rosters are correct and up to date.

• Other duties as assigned.
