Responsibilities and Essential Functions
  • Assess and address patient's needs in the area of housing, food insecurity, transportation or other non-medical areas that influence a patient's health outcomes (Social Determinants of Health). (20%)
  • In partnership with patient care teams, provides support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment. (20%)
  • Assist patients with getting access to social services and other community-based resources. (20%)
  • Follows up with patients via phone calls, home visits and visits to other settings where patients can meet. (20%)
  • Maintain accurate documentation of patient contacts and document patient's needs, action plan and outcomes in the medical record system. (10%)
  • Represents the organization at various community events. (5%)
  • Participate in continuing education, trainings and CHW organizations. (5%)
