How you can make an impact: As a Social Worker at CAMcare, you will work in an environment that supports clinical excellence and competency through multi-disciplinary teamwork and collaboration. The Social Worker will develop a plan for social work intervention that is consistent and congruent with the medical treatment considering the patient’s overall psychosocial status, physical and emotional needs. The Social Worker will establish and maintain a therapeutic relationship with the patient, family and/or significant others and with their participation, formulates social treatment goals that address identified needs or problems and a plan of action for achievement of these goals.

What you will do:

  • Provide social services and assistance to improve the social and psychological functioning of patients and their families.
  • Identify psychosocial status, economic and physical needs of patients; Assesses and reassesses patient’s support systems, available community resources and other factors to plan, develop, and implement an appropriate plan of care.
  • Operate as an advocate with all community populations utilizing skills that respect issues of cultural and ethnic diversity and equity for every patient and family.
  • Coordinate services to Health Center, community agencies and other professionals; maintain working knowledge of and liaison withcommunity agencies; participate in area activities related to health and social services.
  • Assume responsibility for continued professional development; keep current with social work and health care developments and seek to increase further enhancement of job-related knowledge.
  • Assist patients, families, or vulnerable populations with the psychosocial support needed to cope with health, social and psychological concerns.Collaborate with the Health Care Team and involves the patient/family in the development and implementation of plans. Facilitates patient care across a multidisciplinary care continuum which includes interacting with Clinic Providers, and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate plan of care.
  • Provide psychosocial assessments, intervention, and care plan to patients of the Health Center and assists them and their families in coping with their illness, diagnosis, and/or life situation and meet the continuing health care needs.
  • Serve as a patient advocate, maintains a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse.
  • Utilize the EMR system, maintains records and statistics in accordance with department policies.

What you will bring:
  • A passion for helping patients.
  • Master’s Degree in Social Work from an accredited university or college.
  • Licensed Social Work (LSW) OR Licensed Clinical Social Work (LCSW) in New Jersey
  • One to three years’ experience as a social worker in a community health care environment, medical center or other related field.
  • Bi-lingual (English/Spanish) is preferred.
