A health services organization in Connecticut is currently seeking a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) for a great opportunity with their Primary Care team in New Haven County.
Responsibilities:
The Primary Care NP / PA will be responsible for:
- Examine, obtain medical history and treat acute and chronic illnesses
- Order, perform and interpret diagnostic tests
- Create treatment plans, and involve patients and family members in those plans
- Initiate appropriate specialty referrals
- Collaborate on improvement in care delivery and service with management, other Physicians and staff
- Work with the Care team to close care gaps and improve quality measures
- Work with practice management to ensure compliance with billing and coding requirements
- Perform other duties, as needed
- 1+ year of Primary Care experience
- Graduation from accredited APRN or PA program
- CT Nurse Practitioner (NP) and/or Physician Assistant (PA) license
- DEA registration and Controlled Substance certification
- Solid assessment, clinical, and documentation skills
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized
