Position Title: 1.0 FTE Long-Term Substitute Special Education Transition Counselor

Reports to: 9-12 Supervisor of Special Education and the Director of Special Education

Terms of Employment: To cover a leave of absence from approximately August 26, 2024 through November 26, 2024.

$175 per diem for days worked, non-benefits eligible position.

  • Facilitate, plan, and directly support special education students in transition services to prepare them for entering into post secondary life, including vocational skills development, independent living skills development, and college and career readiness.
  • Develop post secondary planning and coordination of the transition process for special education students ages 14 to 22;
  • Provide direct instruction to students in developing vocational, independent living, and college and career readiness skills;.
  • Contribute to organization and planning of professional learning for staff on the subject of transition;
  • Act as Job Coach to students and provides job coach training for support personnel;
  • Collaborate with students, families, and staff on a variety of transition topics
  • Demonstrate expertise and assume responsibility and administration for all transition assessments and their associated timelines;
  • Collaborate with the Guidance Department regarding academic requirements and post-secondary education opportunities for individual students;
  • Conduct active outreach to, and maintenance of, community partnerships with local colleges, employers, collaboratives, outside agencies, adult agencies and other school districts to develop a resource network that will place transition age students in the community;
  • Implement supports and services in keeping with all transition mandates, laws and requirements at the local, state and federal levels
  • Maintain confidential information and data.
  • Participate in professional learning to maintain current with best practices
  • Support diversity, equity and inclusion for all students and families;
  • Other duties as assigned

Minimum Qualifications:

MA DESE license in at least one of the following areas: Teacher of Students with Moderate or Severe Disabilities; Rehabilitation Counselor; School Guidance Counselor; or School Social Worker/School Adjustment Counselor. Seeking applicants who hold, or are working towards, MA DESE Transition Specialist Endorsement. Masters Degree preferred.

History of positive collaboration skills with students, parents and staff; demonstrated strong oral and written communication skills, organizational skills, ability to multi-task and be detail-oriented; Ability to handle complaints, answer questions and resolve disputes in a professional and courteous manner; Effective time management and establishment of priorities; proficiency with technology.
