We are looking for a highly qualified and career-minded Social Services Coordinator to join our social services team and serve residents in our Assisted Living, Skilled Nursing, and Independent Living areas. This is a full-time position.

The Social Services Coordinator is responsible and accountable for assessing the psychological social emotional cultural or financial needs that impact a resident's health and recovery. To provide professional social work services that utilize supportive counseling and clinical knowledge in assessing the needs of patients and families during illness crisis intervention and post discharge planning. Collaborates with members of health care team in the development of a transition plan and addresses needs in a timely manner. Participate in program planning and development to meet the needs of customers within the health care system and community.
