Hospice RN Case Manager

Provides end-of-life nursing care to patients and families primarily at home or in long-term care settings. Coordinates case management of patient services and medical care. Works collaboratively with other disciplines to teach, assist, and support the patient/family with the dying process. As agency needs fluctuate, may be required to work different shifts or locations. Qualifications include current NC or Multistate RN licensure; BSN, CHPN preferred; two years experience in hospice or home health; proficient in electronic documentation; excellent communication, organization, and customer service skills; strong desire to provide compassionate, end-of-life care.
