POSITION SUMMARY:

Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Collaborates with interdisciplinary team to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Utilizes knowledge of patient's age and cultural diversity into the provision of patient care. Under the appropriate supervision as detailed under the LVN Scope of Practice, the Inpatient Wound Care Nurse - LVN will be responsible for the focused nursing care including skin assessment, treatment of wounds, and prevention of pressure injuries.

POSITION EDUCATION/ QUALIFICATIONS:
• Current State Vocational Nurse License (LVN) required
• Current BLS certification.
• ACLS required within 6 months of hire
• Attain Certified Skin and Wound Specialist - ABWH within 6 months of hire
• Strong clinical background in wound care preferred
• Job requires cross training to like areas
• Hospital experience preferred
• Proficiency in English -verbal and written and computer skills required
• Spanish proficiency preferred
• Certifications / competencies related to your work areas

JOB KNOWLEDGE/EXPERIENCE:

Ability to utilize the best nursing process in the provision of nursing care including but not limited to administering treatments and medications, assist formulating a plan of care, collaborating with other health care providers and providing education to patients and/or significant others. The successful candidate will be required to pass competencies for this position.

Essential Technical/Motor skills: Able to adjust and/or troubleshoot monitoring equipment and computer equipment.
Communication Skills: Must be able to interact and communicate with all levels of staff, physicians, patients and families in an effective manner. Exhibits tact, enthusiasm and patience. Must be sensitive to cultural differences.

POSITION RESPONSIBILITES:

Ensures care is delivered to each patient in accordance with the Nurse Practice Act of the State of Texas and ANA Standards of Clinical Nursing Practice.
• Performs family-centered evidence based nursing care through the nursing process of planning, implementation, and evaluation.
• Identify high-risk patients and assists in developing and modifying individualized care plans based on the assessment findings
• Perform regular skin assessments to prevent the development of pressure injuries
• Apply various wound care treatments and therapies
• Assist with the management of wound vacuum-assisted closure (VAC) therapy
• Monitor the progress of wound healing and communicate with RN and physician in adjusting treatment plans as necessary to ensure optimal outcomes
• Educate patients and their families on wound care techniques, preventive measures, and the importance of adherence to the treatment plan
• Coordinate with other healthcare providers to ensure continuity of care and comprehensive patient management
• Accurately document all wound assessments, treatments, and patient progress in the electronic medical record (EMR) system
• Maintain up-to-date records of wound care interventions and patient responses
• Ensure compliance with regulatory and organizational documentation standards
• Apply appropriate wound care dressings as ordered by physician based on the type and condition of the wound
• Educate patients and caregivers on proper dressing techniques and schedules
• Participate in interdisciplinary team meetings and case reviews to offer insights and recommendations for wound care
• Maintain a daily census of all patients receiving wound care, wound vac, consults, and at risk
• Track patient progress, prioritize care, and ensure that no patient is overlooked
• Use of daily census to make informed decisions and allocate resources effectively
• Adapts to the needs of the unit by demonstrating flexibility/adaptability.
• Makes rounds on assigned patients and prioritizes patient's needs.
• Maintains a safe and clean environment for patients and co-workers.
• Administers medication and therapies within the scope of safe nursing practice.
• Assumes responsibility to report clinical limitations or the need for assistance to the supervisor.
• Informs the RN of any anticipated and/or actual issues/problems.
• Maintains current with all new policies and procedures published. Adheres to the policies and procedures that guide and support the provision of services
• Builds a positive work environment by working with others to support the mission of the hospital.
• Flexibility in schedule to meet unit needs and weekend/holiday requirements. Plans and coordinates time schedule with peers to provide coverage.
• Participates in unit based council and/or hospital committees.
• Attends mandatory unit and hospital meetings/classes.
• Abides by the rules and regulations set forth by the Board of Nurse Examiners
• Performs other duties as assigned
