Case Manager/Care Manager Pool- Milwaukee, WI

Community Care is hiring Certified Case Managers/Care Managers who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. We are offering rewarding opportunities to provide care and service in peaceful, tidy and well-supplied residential settings. This is a pool opportunity on all shifts, providing coverage across all Community Care Inc.

Case Manager/Care Manager Job Responsibilities:
• Manages an assigned caseload of program members in collaboration with an RN Care Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Case Manager/Care Manager Job Requirements:

Education: Bachelor's Degree in a Human Services or closely related field required.

Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity required.

Certification: Eligibility for long-term care functional screener certification is required

Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Registered Nurse Case Manager/Registered Nurse Care Manager Job Schedule:

Work Schedule: Monday - Friday; hours of operation (8:00 a.m. to 4:30 p.m.); as needed
