Description

We are growing and are looking to add a Patient Navigator (ARNP or PA) to our oncology team. The Patient Navigator shall be directly responsible for the successful navigation of oncology patients throughout the Florida Urology Partners ecosystem and in collaboration with external care providers. Said navigation shall include, but not yet be limited to patient appointment scheduling and adherence, patient appointment rescheduling, support and facilitation of collaborating with in-office dispensary for oncologic medications processed via Florida Urology Partners, collaboration with Florida Urology Partners Tumor Board and Quality Team, data mining EMR database to ensure follow-up appointments for patients with recent cancer diagnosis, collaboration with Practice Administration on coordination between clinical practices and Oncology centers, as well as other closely-related duties and responsibilities. The Patient Navigator will work closely with the entire healthcare team to ensure that patients receive the best possible care and support.

  • Assists patients in understanding their diagnosis, treatment options and the resources available, including educating patients about appropriate clinical research studies.
  • Provides patients and their families with Florida Urology Partners Oncology Patient guide and treatment planning overview.
  • Serves as the initial point of contact for all oncology patients of Florida Urology Partners.
  • Participates as the essential link between Florida Urology Partners Oncology Team and external Care Providers.
  • Facilitates appointments and consults with Radiation Oncologist and facilitates the supportive services, as required (i.e., dietary, social work, etc.).
  • Collaborates with the multi-disciplinary Florida Urology Partners oncology Tumor Boards for planning, patient summarization, action planning and other care protocols as needed and required.
  • Develop patient education programs and tools.
  • Follow patients throughout the care continuum, including oncology medications, imaging, etc.
  • Work with cancer registry staff, when needed, to collect data, track outcomes, and support strategic planning processes.
  • Deliver quarterly reports to the Chief Operating Officer of Florida Urology Partners across the established quality criteria.
Requirements

Job Requirements:
  • Ability to meet and/or exceed role-specific annual metrics.

  • Ensure a collaborative and efficient experience with high levels of physician satisfaction.
  • Maintain a high attention to detail in the assurance and monitoring of all required documentation across all systems and platforms through various quality assurance checks and internal audits.
  • Effectively communicates status, risk factors, resourcing challenges, barriers to timelines, etc. with Leadership and the Florida Urology Partners Clinical Compliance Committee on a recurring basis.
  • Positively collaborates with Team Members, Physicians, Patients and Visitors, adhering to the Core Values of Florida Urology Partners, LLP
  • Possesses a keen understanding of patient navigation, radiation oncology, urology and other closely-related systems, common oncologic medications, and basic requirements and standards as governed by the State of Florida
  • Strict adherence to following required privacy and security protocols
  • Adept at conflict resolution and solution development
  • Computer proficiency in MS Office and applicable clinical technology (EPIC, e-MDs, Phreesia)
  • Ability to compose quality and professional business communications (reports, emails, letters and memos)
  • Adept in self-directed leadership and the ability to manage multi-disciplinary teams.
  • Bilingual English/Spanish
Professional Experience:
  • Previous oncology, urology, or other closely related functional experience, required
  • At least five (5) years of clinical experience, required
  • Experience with patient assistance programs and case management, required
  • Knowledge of benefit verification and authorization process, required
  • Knowledge of regulatory and compliance requirements per the State of Florida, required
  • Knowledge and understanding of NCI and Centers of Excellence requirements, required
  • Experience in EPIC EMR system, preferred
Education:
  • Active Florida State licensed Advanced Practice Practitioner, or Physician Assistant (PA or ARNP)
  • Bachelor's degree in Business Administration, Public Health, Health Administration or a related field

Experience with EHRs (Epic, eMDs, Qs1)
