Job Summary:

Responsibilities of this position include being the first point of contact for prospective patients, collaboration with our service line and marketing team and management of incoming leads and completing education sessions and scheduling consultations. Provides the maintenance of our patient tracking system, assistance with patient questions and help coordinating appointments required for preparation prior to surgery throughout the continuum of care preoperatively and postoperatively up to 3 months. The navigator will be responsible for regular follow up with patients, monitoring progress,

programmatic patient coaching and continuum of care to the postoperative phase. Communication is handled in person, by phone, email and within the EMR system. Navigators work with the clinical teams and providers to identify patient who are ready to proceed to surgery. Partnership with management on metrics, reporting, and patient experience is included.

  • High School Diploma or GED
  • Certified Clinical Medical Assistant Or Registered Medical Assistant Or Certified Medical Assistant
  • Minimum 2 years of experience working in a Physician Office or Clinical Role in a healthcare setting.
  • Completion of PSS Epic Training.
