Key Responsibilities:

  • Plan, organize, and direct home care services to meet patient needs.
  • Conduct thorough assessments of patients’ healthcare requirements.
  • Develop and implement personalized care plans in collaboration with care teams.
  • Counsel patients and family members, providing education and healthcare instructions.
  • Prepare clinical notes and effectively communicate patient conditions to primary physicians.

What We’re Looking For:

  • Licensure: Valid Arizona State Board of Nursing RN License.
  • Experience: At least one year of experience in home health care or hospice preferred
  • Transportation: Valid Arizona  Driver's license and reliable transportation.
  • Certifications: Current CPR certification and negative TB test (we can help you with this)
  • Background Clearance: Satisfactory fingerprint clearance per federal/state regulations.
  • Bilingual Preferred: Conversational Spanish skills are highly desired.

Education:

  • Associate’s Degree in Nursing or higher learning
